Um HD 11 novinho em folha para fazer exames de...
Camundongos!!!!
Vou pilotar essa máquina com um transdutor linear de 15 mhz para fazer eco bidimensional, doppler e 3D de bichinhos de 25 gramas.
Não estranhem se as próximas fotos que aparecerem nesse blog forem de pequenas cobais.

There are two fundamentally different approaches to assess
CAD, functional and anatomical. From a functional perspective,
a haemodynamically significant coronary stenosis
renders coronary blood flow inadequate to meet the metabolic
requirements of the dependent myocardium at rest
or during stress, or both. This condition is referred to as
reduced coronary flow reserve. However, which morphological
features on anatomic imaging such as SCA or CCTA are
associated with a trans-stenotic gradient is controversial.
There is general consensus that coronary artery stenoses
exceeding 70–75% of the reference diameter are likely to
be haemodynamically significant and may warrant coronary
revascularization. However, in the presence of ‘borderline
coronary stenoses (50–60% diameter in locations other
than the left main coronary artery)’, choosing coronary
revascularization as a therapeutic strategy is supported by
evidence only if there is ‘demonstrable ischemia on noninvasive
testing’.6,12 The reason for this stipulation in the
ACCF/AHA guidelines is that luminal diameter stenoses
between 50 and 70% may not reduce coronary flow reserve
as consistently as stenoses of 70%.13,14 Correlative clinical
studies between invasive, selective angiography and exercise
ECG have therefore been divided on the use of 50 or
70% luminal narrowing to define significant stenosis.4
While studies comparing CCTA with SCA have generally
considered 50% diameter stenoses as ‘significant’,3 this criterion
may not be ideally suited for comparing anatomical
and functional testing for the detection of CAD. The discordance
in diagnostic accuracy between functional testing and
CCTA results in the current study and others15–17 is therefore
not unexpected.
Is there a role for complementary non-invasive functional
and anatomical assessment? Preliminary studies of integrated
hybrid imaging combining CCTA and myocardial perfusion
imaging for the assessment of CAD have
demonstrated promising results,18 but the potential benefits
must be weighed against the substantial radiation
exposure.19 Prospective studies, such as the Study of Myocardial
Perfusion and Coronary Anatomy Imaging Roles in
CAD (SPARC) registry,20 that are designed to determine the
prognostic value, cost-effectiveness, and associated risks
of CCTA, hybrid imaging, single-photon emission tomography,
and positron emission tomography, are ongoing and

work-up of patients suspected of having CAD.

Conclusão:Ecostress com exercício ( 26.295 pacientes!) é mais seguro que Dobutamina e Dypiridamol.

Massa AE com contraste = mixoma



Veja que didático esse caso:
1- Imagem de massa atrial esquerda
2- O contraste acaba de chegar ao VE mas ainda não perfunde a massa.
3- O contraste perfunde a massa, mostrando existir circulação.

Achado operatório de Mixoma.

Em caso semelhante no HC UNICAMP, também observamos o tempo decorrido para as microbolhas serem "lavadas" da massa, em relação ao musculo vizinho.
Um "wash out" muito rápido seria sugestivo de massa maligna.

E também o contrário, sem perfusão da massa AQUI

Conclusions— Serial angioscopic findings up to 2 years after SES implantation were markedly different from those after BMS. Neointimal coverage was completed by 3 to 6 months in BMS. In contrast, SES demonstrated the presence of thrombi and yellow plaques even as much as 2 years after implantation.

TEMPO PELE-TRANSDUTOR COMPLETO



Atendo agora alguns pedidos de publicação na íntegra do tema tempo pele-trnsdutor.

Essa semana o assunto volume de ecos foi freqüente. Enquanto um colega contava ter feito 30 exames em uma manhã com orgulho, outro colega criticava um terceiro por fazer 600 exames por mês!
Existe uma diferença entre rapidez e eficiência que permite fazer exames com qualidade.
O fundamental é não mexer no tempo de transdutor na pele, o exame com imagem deve durar o tempo necessário para o diagnóstico.
Agora,existe uma larga margem de tempo a ser cortado entre um exame e outro,
de modo que, sem mexer no tempo pele-transdutor, pode-se ir de 2 para 5 exames por hora!

O mais importante item da economia de tempo, ou otimização dele, é o sistema de laudos.
Como 80% dos laudos têm poucas ou nenhuma alteração, um laudo desses deve ser digitado em menos de 1 minuto.
Quem deve digitar é o médico, a secretária estará ocupada colocando outro paciente!

Faça as contas, 1 minuto por laudo, 25 laudos por dia, 25 minutos perdidos ou dois exames a menos ou 250 reais a menos por dia ou 1250,00 por semana! Em 1 ano pode deixar de ganhar 60.000,00 reais!!!!!
Comece por um laudo rápido e bom.
Quanto tempo é gasto para o paciente chegar à sala, colocar o avental, deitar na maca e após o exame, trocar-se e permitir a entrada de outro paciente?
Sendo mulher idosa em um laboratório mal posicionado, pode ser de até 10 minutos!

O laboratório ideal tem duas salas de troca com entradas independentes, assim um paciente pode já estar trocado quando o outro sair.
No mínimo deve ter um trocador isolado da sala de exames.
O médico dá o laudo na sala de exames enquanto o paciente se troca, uma sala de LAUDOS atrasa o sistema.
Estude o mecanismo de troca de pacientes em seu serviço, pequenas mudanças de baixo custo podem aumentar sua produtividade em até 50%(experiência própria!)
Um capítulo da otimização do tempo está relacionado à impressão. Não apenas à impressão mas a captura de fotos e sua entrega no formato desejado (impressa, CD ou Disquete)

Meu esquema ideal envolve a captura de fotos no próprio aparelho e impressão em Laser colorida. É rápido, de qualidade e usa o convencional.
Com o computador ao lado do aparelho de eco, já coloco as medidas no laudo ao realizá-las.
Alguns serviços usam gravação em CD.Também é rápido mas custa 60 centavos a mais por laudo e incomoda quem recebe o exame.
Impressão com jato de tinta, nem pensar, com cera então, só se seu serviço fizer mais de 1 000 exames por mês.

O importante é que o processo leve menos de 1 minuto, invista em uma laser HP boa e seja feliz!
Chegue na hora! Não se atrase!

Uma mágica simples para aumentar a produtividade sem interferir na qualidade é chegar na hora.
Marcou para as 8:30? Chegue as 8:25!
Quem começa na hora tem muito mais chance de terminar sem reclamações um período de exames. O atraso incomoda a secretária, que perde a confiança em agendar e encaixar exames, e incomoda o paciente.

A cultura do atraso do médicos não cabe no setor de serviços diagnósticos...
Como o agendamento pode ajudar na produtividade?
Para o paciente, esperar é sempre ruim. Um bom agendamento pode evitar atrasos e melhorar a imagem da clínica.
Pense em marcar, por exemplo, os primeiros horários de 15 em 15 e a segunda metade da agenda de 20 em 20. Como os atrasos são cumulativos, dê maior margem para manobra na segunda metade da sua agenda.
Faça os encaixes no horário mais folgado e faça encaixes!
É bastante útil fazer um fluxograma da sua clínica, se o pico de movimento é na quinta, como a nossa, ofereça um horário amplo nesse dia, como horário de almoço e começo da noite.
Lembre-se, no setor de serviços, o horário deve ser ideal para o paciente, não necessariamente para o médico e secretárias.
Sendo a terça mais calma, diminua a agenda e vá para a academia, ecocardiografistas têm que fazer musculação e alongamento para ombro!

AVALIAÇÃO DIASTÓLICA - VALE A PENA LER!

Conventional Echocardiography
It is generally accepted that Doppler echocardiography cannot provide unequivocal evidence of diastolic dysfunction in HFNEF. The E/A ratio, IVRT, DT, and pulmonary vein Doppler16 characterizing flow across the mitral valve do not allow direct measurement of LV relaxation, stiffness, or filling pressure.15,17 Several authors have demonstrated that conventional Doppler is accurate in patients with a reduced EF but not in those with normal EF.16,35 We also found only a weak correlation between IVRT and in our study population, in agreement with others.36,17 Furthermore, a short DT indicates increased LA pressure in patients with systolic37 or pseudonormal and restrictive diastolic heart failure.15 In contrast, patients with a mild diastolic dysfunction have a prolonged DT and therefore reduced K. Thus, with regard to their biphasic response to increasing diastolic dysfunction, the interpretation of E/A or DT in diagnosing diastolic dysfunction is rather complicated. Although the mitral inflow parameters DT and K correlated with b (dP/dV), simple analysis of E/A or DT was limited in at least our study population, which was patients with impaired mitral flow. This limitation is further underscored by our finding that these parameters were not significantly related to ß, known to be a relatively load-independent parameter. If we had used the mitral flow Doppler as the only technique for detecting diastolic dysfunction in HFNEF, 70% of patients would have been correctly identified. Although all 5 patients with pseudonormal mitral flow pattern had been identified correctly in our study, indicating that the mitral Doppler is a helpful diagnostic tool in cases of more severe diastolic dysfunction, we conclude that it is of only limited value in the diagnosis of early diastolic dysfunction in HFNEF. We observed that the duration of atrial reverse flow was significantly prolonged, an early sign of disturbed mitral inflow resulting from increased LV stiffness. However, the limited diagnostic accuracy and signal quality limited the practical use of pulmonary vein Doppler in our study. A combined analysis based on E/A and IVRT, DT, or Ar–A duration improved the accuracy of the mitral flow Doppler method, but only to a moderate degree. In summary, filling pattern analysis from mitral flow Doppler measurement alone is found to be more complicated and limited to detecting early diastolic dysfunction in patients with HFNEF.

Tissue Doppler Imaging
TDI proved to be more accurate than conventional Doppler for detecting impaired diastolic function in patients with HFNEF.29,36 In general, we found that the lateral annular velocities were more closely related to the LV relaxation and compliance indexes as determined by PV-loop analysis than the septal annular velocities (Table 3). Thus, only the lateral velocities are taken into consideration in the following discussion. With regard to impaired LV relaxation, we confirmed its relation to the early diastolic mitral annular velocity (E’)18 and TE-E’.30 However, the latter did not correlate with the filling pressure, as previously suggested.30 We found that the TDI indexes E’lat and E’/A’lat correlated more closely with LV stiffness than any conventional echocardiography index. Similarly, the dimensionless E/E’ index, introduced recently as an echocardiographic measure of LA pressure and LV filling,2,16,35,38,39 showed the best correlation with indexes of diastolic parameters obtained by PV-loop measurements. In our study, patients with HFNEF and E/E’lat >8 had a significantly increased LV stiffness (Figure 1). Both E/E’lat >8 and E’/A’lat <1 detected HFNEF patients with diastolic abnormalities equally well, but E’/A’lat showed lower sensitivity, yielding more false-negative results than E/E’lat. Because we did not perform PV-loop and echocardiographic investigations simultaneously, however, we found only rather moderate correlations in our small study population. Nevertheless, from a clinical point of view, the key question is whether the echocardiographic method used allows reliable detection of the correct diagnosis. In contrast to Doppler echocardiography, TDI detected diastolic dysfunction in 81% (35 of 43) and the E/E’lat index in 86% (37 of 43) of our patients with HFNEF. Three additional patients with HFNEF were identified by adding E’/A’ to E/E’lat, raising the detection rate to 93% (40 of 43). In contrast, the additional application of conventional Doppler indexes did not considerably improve the diagnostic accuracy.

Some recent studies40–42 reported reduced regional systolic peak velocities in patients with HFNEF and impaired systolic reserve,43 suggesting that systolic function also is impaired. Our study has not confirmed this finding. In addition, our invasive catheter measurements showed that global systolic function and contractility of the patients with HFNEF were not impaired under basal condition, in agreement with others.1,19,44,45 Compensatory capacities and/or systolic reserve in our relatively young study population may have contributed to a limited difference in systolic parameters. However, further studies under stress conditions are needed to further clarify the role of systolic function in patients with HFNEF.

In summary, in clinically stable patients at rest presenting with reduced exercise capacity in whom the diagnosis of diastolic dysfunction was proven by conductance catheter analysis, single indexes of conventional Doppler echocardiography were insufficient or inferior compared with TDI parameters in detecting the correct diagnosis. Although the diagnostic accuracy improved after several indexes of the mitral and pulmonary venous flow analysis were added, we do not recommend their use as isolated method for investigating diastolic function, which is in agreement with the latest consensus statement of the Heart failure and Echocardiography Association of the European Society of Cardiology.46 In contrast to flow Doppler, TDI parameter showed better linear correlation with diastolic parameters and provided a simple means of diagnosing diastolic dysfunction. Accordingly, TDI was a more reliable technique to identify early disturbances of both LV relaxation and stiffness. However, although the LV filling index E/E’lat showed a similar sensitivity but higher specificity than E’/A’lat in detecting diastolic dysfunction, we recommend the use of E/E’lat in both clinical diagnostic routine and scientific studies to investigate diastolic function in patients with HFNEF.

Conversando com uma colega há um ano, ela me revelou que após estudar muito Doppler tecidual, dominava a técnica e uso...
Nos EUA ainda não têm ninguém assim, mas eles continuam tentando.

ELES estão chegando...

Conclusion: Non-invasive CTCA is a potentially useful tool, in the diagnostic work-up of patients with typical angina pectoris, both to detect and to exclude significant CAD.

Feito o exame com contraste e adenosina, a porção apical antero-septo-lateral tem perfusão indetectável.
Isquemia!

Cateterismo feito 4 dias depois: Lesão proximal DA de 70%, Lesão 80% CX após 1mg.

A paciente tinha dor atípica há 1 ano, com 76 anos estavam optando por adiar o cateterismo.

Da Aterosclerose à Aterotrombose 10



Como cresce uma placa?
Parece que ela sangra, cresce, sangra, cresce, 3, 4 vezes antes de apresentar uma estenose de 70%!
No estudo acima, as rupturas com sangramento foram evidenciadas e comparadas ao grau de estenose.
Mesmo as placas "duras" são formadas por rupturas sequenciais de placas "moles" na grande maioria das vezes.
